Performer Agreement, Acknowledgment & Liability Release

Thank you for being willing to serve as a Community Liaison during the Psychedelics in the Beehive experience!

This form serves as your official contract agreement with SaraU Productions for you to facilitate a meet-up performance on Friday or Saturday, January 24th or 25th. Please complete it by providing your digital signature, and you will receive a copy of the signed agreement for your records once submitted.

Your primary contact for any event-related inquiries will be Erin Weist and if you have any additional support needs, please reach out to Andy Silva at andy@ourrevival.org. We look forward to working with you and sharing an unforgettable show!

Services & Compensation

Description of Services (Performance): You agree to facilitate a meetup of a subgroup of this community. Date/Time of your meetup: Friday or Saturday January 24th or 25th 2025. Location Provided by Organizer: - Location will be in the Gateway (actual storefront TBD) Compensation: -In exchange for your participation, you will receive a ticket to the event. -All tickets need to be claimed in December. -In January, you will receive an email indicating a firm time and location for your meetup.

Promotion Obligations

You agree to make at least 4 promotional posts about the event on your social media platforms. At least 2 of these posts should be made more than 30 days prior to the event.

Use of Image and Media Release

By participating, you grant Psychedelics in the Beehive and SaraU Productions permission to use your name, likeness, images, video footage, and sound recordings of your performance for promotional purposes, including future events and marketing.

Liability & Conduct

Attire, Conduct, and Entertainment Act Compliance: You agree to abide by the Attire, Conduct, and Entertainment Act. Key points include: -Ensuring performances do not involve illegal activities or violate city ordinances. -Refraining from nudity or acts deemed obscene under state law. -Maintaining a professional demeanor and following conduct guidelines for performers. -Observing all local alcohol and licensing regulations related to entertainment. No Illegal Substances: You acknowledge that you are not to solicit, promote, sell, or distribute any illegal substances at the event. Artistic Interpretation Notice: You understand and agree that your performance is an artistic interpretation of psychedelic experiences and that you will not encourage illegal behavior or substance use.

Conduct & Contractor Status

Independent Contractor Status: You confirm you are providing services as an independent contractor, not as an employee, agent, or partner of SaraU Productions or Psychedelics in the Beehive. Hold Harmless: You agree to hold harmless and release SaraU Productions, Psychedelics in the Beehive, and their agents from any liability for injuries, damages, or losses related to your participation. Professional Reputation & Cancellation: The Organizer reserves the right to cancel this agreement due to scheduling conflicts, budget restraints, or adverse changes to your professional reputation. Psychedelics in the Beehive maintains a zero-tolerance policy for working with anyone who has credible claims of sexual assault or harassment voiced against them, and the Organizer may sever the contract immediately if such claims arise. Sobriety: You agree to remain sober while fulfilling your performance obligations.

Governing Law & Entire Agreement

-This Agreement is governed by the laws of the State of Utah. -This Agreement represents the entire understanding between the parties and supersedes all previous agreements, oral or written.

By typing in your full legal name, you are entering into an agreement with SaraU Productions and Psychedelics In The Beehive. This Agreement constitutes the entire understanding between the parties and supersedes all prior discussions, negotiations, or agreements, whether oral or written.
